This tutorial and the accompanying review (Helm, 2004) provide an example of how to apply the concepts presented in EBP Tutorials #1 through #5 (Hargrove, 2002a, 2002b, 2003a, 2003b;  “Tutorial: Evidence-based practice #1,” 2002). To facilitate interpretation of the EBP Review Form, the tutorials explaining items on the form are referenced. Although it is not necessary that both the form and the review be completed, together they provide a comprehensive analysis because of their different, yet complimentary, purposes. Forms such as the EBP Review Form ensure a thorough evaluation of research, and prose—such as Helm’s review— allow for the integration of findings. The combination of the form and prose review is particularly useful for use in journal groups in which members share EBP reviews among themselves.
